What Is the Skill Performance Evaluation (SPE) Certificate Program?

The FMCSA Skill Performance Evaluation (SPE) Certificate Program helps commercial drivers with limb impairments prove they can drive safely. Drivers with missing, impaired, or paralyzed limbs may still qualify for a DOT medical certificate if they meet FMCSA standards.

Who Needs an SPE Certificate?
You need an SPE certificate if you have a missing or impaired hand, arm, foot, or leg that affects your ability to operate a commercial vehicle (CMV). The FMCSA requires this evaluation to ensure safe driving ability.
How to Apply for an FMCSA Skill Performance Evaluation (SPE) Certificate
- Get a Medical Evaluation
Visit a DOT-certified medical examiner to assess your overall fitness to drive. Schedule your exam at E-Z DOT Physicals at 84 Chiropractic. - Complete a Driving Test
You must pass a driving skills test using a vehicle with proper modifications. - Submit an Skill Performance Evaluation (SPE) Application
The application requires:- A DOT physical exam report (Form MCSA-5875)
- A Medical Examiner’s Certificate (MEC)
- A description of your limb impairment
- Proof of driving ability
- Completion of the New Driver Application Package as outlined by the FMCSA. Download the application package here.
- FMCSA Review and Approval
The FMCSA evaluates your medical fitness and driving skills before issuing the certificate.
What to Expect During the Driving Test
The SPE driving test ensures you can safely operate a commercial motor vehicle with your impairment. The examiner checks:
- Steering, braking, and shifting ability
- Use of adaptive equipment (if required)
- Ability to perform emergency maneuvers
Maintaining Your FMCSA Skill Performance Evaluation (SPE) Certificate
- The SPE certificate is valid for two years.
- You must carry it with you at all times while driving.
- You must complete a new DOT physical and SPE renewal before it expires.
- Any changes in your medical condition may require re-evaluation.
Can You Get a DOT Medical Card Without an SPE Certificate?
If you have a limb impairment but do not meet SPE requirements, you cannot pass the DOT physical. However, some intrastate driving jobs may have different requirements.
