Commercial drivers with diabetes, whether insulin-treated or not, can still pass the DOT physical. Understanding the rules and preparing ahead of time will make the process smoother. This guide explains the steps for drivers with both insulin-treated diabetes mellitus (ITDM) and non-insulin-dependent diabetes mellitus (NIDDM). Let’s walk through the key information that will help you pass your exam.

Key Requirements for Drivers with Insulin-Treated Diabetes Mellitus (ITDM)

If you have insulin-treated diabetes, the process is slightly different from drivers managing diabetes without insulin. Here’s how to navigate the DOT physical with ITDM.

  • 1. Obtain the MCSA-5870 Form from Your Treating Clinician
    • Your Treating Clinician, who manages your insulin treatment, needs to complete Form MCSA-5870, known as the Insulin-Treated Diabetes Mellitus Assessment Form. This form confirms that:
    • Your insulin regimen is stable.
    • You are controlling your blood sugar levels properly.
    • Your treating clinician must provide this form to the certified Medical Examiner, who will use it during your DOT physical. Without this form, the medical examiner cannot complete the exam. Make sure you have the form ready before scheduling your exam.
  • 2. Submit the Form within 45 Days
    • Once your Treating Clinician completes the MCSA-5870, you must give it to the certified medical examiner. You have 45 days to submit the form, and the ME must start your exam within this window. However, the medical examiner does not need to complete the certification process within these 45 days.
  • 3. No “Determination Pending” for Missing Forms
    • Be aware that if you arrive at your DOT physical without the completed MCSA-5870 form, the ME cannot use “Determination Pending” to give you more time to get the form. You’ll need to reschedule the exam if you don’t have it in hand.
  • 4. Certification is Valid for Up to 12 Months
    • Once you pass the DOT physical, your certification will be valid for up to 12 months. However, this certification period depends on maintaining proper control of your diabetes.
  • 5. Give 3 Months of Blood Glucose Monitoring Records
      • To qualify for the full 12-month certification, you must give three months of blood glucose monitoring records to your treating clinician. These records must be electronic (not hand-written). You’ll need to show that you’ve been regularly checking your blood glucose levels, although the FMCSA does not need a specific frequency for testing. If you can’t provide three months of records, the medical examiner may grant a temporary certification for up to three months, allowing you to gather the required records.

    Passing the DOT Physical with Non-Insulin-Dependent Diabetes

    For drivers with non-insulin-dependent diabetes mellitus (NIDDM), the DOT physical process is more straightforward. You won’t need form MCSA-5870, but you’ll still need to demonstrate that your diabetes is well-managed. Here’s what to focus on:

    1. Control Your Blood Sugar Levels

    Keeping your blood sugar in a healthy range is key to passing your DOT physical. Regularly monitor your blood glucose levels and take any prescribed medications as directed.

    2. Maintain a Healthy Lifestyle

    A balanced diet and regular exercise can greatly improve your chances of passing the DOT physical. Avoid high-sugar foods and stay active to support your blood sugar control.

    3. Bring Necessary Medical Records

    While you don’t need an MCSA-5870 form, you should still bring your medical records, including:

    • Recent blood sugar logs
    • A1C test results
    • A list of any medications you take for diabetes

    These documents will help the medical examiner understand your condition and determine whether you meet the medical requirements for commercial driving.
